How to make a referral

A referral must be made in writing by emailing our admissions Manager at admisions@expediuscare.com. If you would like to discuss the referral before completing our application form the Admissions Manager can be reached on +44 123 456 7890

The referral must include relevant information (such as age, gender, back ground and safeguarding concerns) about the young person to help us undertake our initial needs assessment.

We have provided a referral form that can be downloaded with this link. Completing this form will help streamline the referral process and allow us to efficiently gather the information we require for our initial assessment.

Referral Process

Our Admissions Manager carefully reviews all referral request, undertakes a risk assessment related to the young person’s needs, and reviews the suitability of all our available homes taking into consideration the needs of the existing service users in the homes. The young person is then matched with the right accommodation and a placement offer is made.

From the outset of the referral process, we seek to consult all relevant parties in the needs and risk assessment process.

Confidentiality: We understand the importance of privacy and confidentiality. All information shared during the referral process will be handled with the utmost discretion.
